Henrico County, VA -- The Virginia Department of Labor and Industry has cited a Pennsylvania-based company for eight violations that killed two construction workers.

On November 1 two construction workers were salvaging materials inside a warehouse that used to be a large printing company.  The men were cutting pipes that used to have printing ink on them.

It is believed that the tools created an ignition source for the chemicals and a flash fire ignited, burning two construction workers.

The citations totaled $17,880.  The citations include

  • Employer did not have a person with a valid certificate in first aid training available on the job site.
  • Employer did not determine if dangerous chemicals were on the pipes before cutting the pipes.
  •  Two employees were not wearing full body harnesses attached to the boom or basket.
